DEA
Each state you work in requires a federal DEA with an address in that state. You can change an address on an existing DEA that is no longer in use or apply for a new one using this link. The DEA application confirmation can be used until the official DEA is issued. Please email or fax this document to us.
Note: A state license is required in the appropriate state before you can apply for a DEA for that state.
When applying for the DEA in MN or SD, if you don’t have a practice or home address in the appropriate state, you can use 815 12th Street North, Virginia, MN 55792 in MN and/or 603 S Dakota St, Milbank, SD 57252 in SD. Each of these addresses is checked regularly by Wapiti staff and the information will be forwarded to you. When using these addresses, be sure to enter your home address as the secondary address so all renewals will be sent directly to you.
